TruTrak Servo SB - Arm retention

Some years ago, before TruTrak sold to BK, there was a service bulletin about retaining the arm to the servo. There's also a long thread here on VAF with possible solutions, and the eventual SB. [https://vansairforce.net/threads/tru-trak-servo-arm-belt-and-suspenders.57487/page-3] As I recall the solution was a clip like part that wrapped around the arm lengthwise to retain the screw. I can't find any photos of that 'clip'.

I'm looking for that as BK support says "unsupported, you need to replace the servo", and has taken down all the info. Aside: frankly that seems like liability exposure given that one case involved jammed controls in flight.

Anyhow I'd like a good photo so I can fabricate the part. Anyone have an image? I'd also like a copy of the TruTrak SB that had the image.

The head of the screw rests in that hole, but is too big to actually fall through the hole if it (the screw) begins to back out. Maybe it was easier than putting a recess in the bracket to sit on the screw head like a glove.

I should note...the pictured bracket is on the Trutrak B servo that's used for roll. I don't know if other servos are physically similar.

The pitch servo arm on a DSP-B is the same as on the DSB-B roll servo. For reference to future readers. However I find that putting a longer arm on the roll servo, when locating it in the under seat spot on the 6, is needed. Only modification to the 'clip' would be to make it longer in that case.
 
The clips were the same both for pitch and roll servos (Digiflight series).
Need to look for a picture, but remember they were quite easy to install/retrofit.
