What documents do we need to support a special flight permit request

Our plane may have the Condition Inspection expire (end of May) before we can pick it up from the paint shop (much delayed not poor planning on my part).

I was looking at the AWC portal and filling out the request. What documents are needed to be attached in the Documents tab? I see there is a partially completed 8130-6. Do I need to print that out and sign it and re-attach it? Anything else?

One of my local FSDO "friends" and I had a discussion about ferry permits for experimentals, and he said he had never issued one, and didn't believe it was an FAA requirement. I have applied/received several for certified aircraft , as an AP/IA I had to validate items where satisfactory and lay out the details of the flight. As we all know, different FSDO's have different opinions and expertise . I'm interested hearing other historical facts on this. At the time, I was looking into moving and EXP that had not completed the issued phase one hours, and the FSDO required the area be changed to meet the flight/movement. We did that, but it took a DAR and a long time...so much that we pulled the wings and trucked it to the new location.
 
I got one last summer because my family moved across the state (out of my Phase I area) before I finished testing, and it wasn't going to be feasible to finish the testing in any reasonable time.

I had to first fill out the standard special flight permit form. Then after a lengthy discussion with an FAA person (who had never dealt with experimentals before and was confused at first by the request), I had to provide copies of my AWC and registration, get the aircraft signed off by an A&P (because I didn't have my repairman cert yet) and provide a map of the proposed route of flight.

Then, after making the flight I got to (again!) go through the process of getting an amended AWC for a new Phase I area, and all the trouble that Phase I areas now are. Used my DAR's services again for this one...
 
Chapter 18 in order 8130.2L covers the issuance of Special Flight Permits.

In a nut shell since the chapter is quite long and most of it not relevant to to a permit request for the purpose of repositioning an aircraft -

Submitted Documentation - to show that even though it may not meet current flight status requirements, it is in a condition for safe operation for the purpose of the requested flight. This can be a maintenance log entry made by a person that meets the requirements ;listed in the operating limitations regarding who can complete a condition inspection (Orig builder with repairman certificate or A&P).

You do not need to print and sign the 8130-6 form that is present in the documents list. The AWC system is producing a digital copy of an 8130-6 while you are entering all of the required information, and it will provide you with an opportunity to digitally sign it at the end before you click the submit button.

Thanks Scott, so I just need to attach the maintenance log entry to the AWC portal? Would attaching the registration or airworthiness certificate be required?

It has been a while since I read through the requirements in Chapter 18, so if you do not wish to do so your self, including them wouldn't hurt.
