Aircraft B3 import form

Hi all. I bought an RV8 in the US and planning to fly to Canada soon. Does anyone here have experience filling out the B3 import form? An example would be much appreciated if someone has one filled out.

I imported my RV-6 about 10 years ago and will try to help if I can. I'm not familiar with a "B3 form" though?
